For me (can't say it is the solution for everybody) it turned out to be a problem with the PHP caching plugin APC. Switched to eAccelerator and it worked like a charm. My current host is a bit slow, so I don't know if it makes a lot of difference, but my YSlow went from E to B with only PHP Speedy responsible for it! I do recommend it yeah.
